About the Program

The Kadalyst Wellness Program is an annual program that encourages employees to engage in educational activities, goal-setting, and examination of their current lifestyle with the goal of developing good habits for holistic healthy living. The program is internally supported by a committee of employees from Kadalyst. It is our hope that this program will offer you the opportunity, encouragement, and incentive to pursue and form new healthy habits in your own life.

There are requirements to the wellness program to develop and reinforce good habits that promote individual health and wellness, as well as increase one's knowledge of the impact of personal holistic wellness.

Current program year: 1/1/17 – 12/31/17
Employee incentive: premium coverage at 95% employer contribution

1. Opt-in Form
2. Waiver of Liability Form
3. Preventative Care Appointment
4. A minimum of 25 Activity Points

How to Log Your Activities

1. Click on a category from the Main Navigation
2. Click on an activity to submit verification
3. Select "Choose File" to include an attachment
4. Select a file to attach to the activity
5. Click "Submit"
6. Upon submission, your verification goes into review by Kadalyst
7. Once approved your badge will be displayed

"Free Choice"

If you physically cannot complete an activity, you have "Free Choice" options you can use to replace any activity that you cannot or do not want to complete.

Verification

Each activity requires some form of verification that it is complete. Most activities will have a tracker, but you can use a photo, an email from HR, a note from your supervisor, a receipt/registration form or your own tracker/log.
